Request for Counseling Confidential Application Form

The Michigan Small Business & Technology Development CenterTM has been established to provide consulting, training, research and other services to small businesses. There is no charge for consulting sessions; however, the attached application must be completed and provided to the MI-SBTDCTM prior to receiving services. There may be a charge for costs incurred in the process of consulting or other services that are not part of the normal resources that the MI-SBTDC provides. No fees or financial obligations will be incurred by the applicant without their prior knowledge and agreement.

You are encouraged to look at these application forms closely, and if desired, you may review them with your legal counsel or other business advisors. With the exception of name and address, all information submitted here and future information that may be provided to facilitate the consulting process is exempted from the Freedom of Information Act. However, MI-SBTDC, SBA, and state audit agencies may contact you to confirm that assistance was provided.
